Architectural and space-planning solutions

The project documentation on the site provides for the construction of a polyclinic building for adults and children with a total capacity of 340 visits per shift (adult department with a capacity of 220 visits per shift; children's department with 120 visits per shift), a geriatric department with a capacity of 50 visits per shift, a women's consultation with a capacity of 50 visits per shift and pharmacy. The building is 2-3 storey, a basement floor is designed under the central part of the building, a basement and a technical underground are designed under a part of the building. The building has a complex shape in plan with dimensions in the axes of 66,0 x 81,0 m. The height of the floors is 3,9 m, the height of the basement floor and the height of the basement floor is 2,70 m, the technical underground is 2,37 m. The height of the main volume of the building from the planning mark to the top of the parapet of the 3-story part is 14,10 m, the maximum height of the protruding volumes of staircases and ventilation chambers from the planning mark to the top of the parapet is 16,90 m. children's department, to the children's department for infants, to the geriatric department, to the antenatal clinic, to the pharmacy and separate entrances for staff. The premises of the pharmacy are designed on the ground floor. The entrance of visitors to the trading floor of the pharmacy is provided only through a separate external entrance. The basement floor is designed for: a central sterilization room, cloakrooms and utility rooms for the staff of the polyclinic with a canteen for staff, technical rooms. Entrances to the basement are provided through pits. On the 1st floor, vestibule groups of entrances to the polyclinic departments, including the antenatal clinic, with cloakrooms for visitors, a reception desk and other premises were designed. Doctors' offices are designed on floors 1-3. The premises of the pool with a bathtub 5 x 12 m for children aged 10 to 14 years for 10 people with showers and dressing rooms are designed on the 1st floor. The administrative premises of the polyclinic, accounting, a conference room for employees with 108 seats are located on the 2nd floor. On the 3rd floor, a day hospital for 14 beds (five wards for 2 and 4 beds with bathrooms at the wards), a department of physiotherapy and exercise therapy, rooms for functional diagnostics, and a radiation diagnostics department are designed. For vertical connection between the floors, six stairwells are provided with access directly to the outside. Two cargo-passenger elevators with a carrying capacity of 1000 kg have been designed. Bathrooms for visitors, bathrooms for disabled visitors and bathrooms for staff are provided on all floors of the building. The covering is flat, combined, with an organized internal drain. The roof is rolled, separate sections of the roof are protected by a layer of paving slabs. The height of the parapet or fence is at least 0,6 m. Light lanterns were designed above the pharmacy trading floor and above two three-light halls on the 1st floor. There are exits to the roof from the stairwells. Ventilation chambers are designed on the roof. Filling of window openings - metal-plastic window blocks with double-glazed windows. External finishing of facades: plinth - lining with artificial stone, walls of above-ground floors - finishing of two types: facade plaster and porcelain stoneware finishing according to the ventilated facade system. Partitions - brick (in the premises of the pool - solid brick), plastered on both sides. Interior decoration is designed in accordance with the functional purpose of the premises. Measures are envisaged to ensure the life of the disabled and other low-mobility groups of the population. Five entrance porches for visitors to the departments of the polyclinic are designed with ramps with an 8% slope for climbing to the level of the first floor. To descend to the level of the pharmacy entrance platform in the basement, an inclined lift is provided on the outer staircase. Designed fire safety zones on the 2nd and 3rd floors.

 Structural and space-planning solutions

The building is designed according to the frame structural scheme of monolithic reinforced concrete and is divided by expansion joints into 3 blocks. Building responsibility level – II. The spatial rigidity of the building is ensured by the joint work of vertical load-bearing elements (frame columns and stairwell walls) with unchanging horizontal disks of reinforced concrete floors and roofing. Frame columns - with a section of 4040 cm, the main column spacing is 6,0 x 6,0 (3,0) m. The load-bearing walls of the stairwells are monolithic reinforced concrete with a thickness of 200 mm. Ceilings and roofing - slabs of monolithic reinforced concrete 200 mm thick along cross beams 400x500 (h) mm and 400x600 (h) mm (for a span of 9,0 m). Covering structure in the area of ​​the central entrance - roofing sandwich panels on steel girders from channels and beams of the I-section covering. Steel C255. The outer walls of the basement floor are made of monolithic reinforced concrete 200 mm thick, with a layer of insulation, and, above ground level, with wall stone cladding. Concrete of monolithic structures of the above-ground part of the building B25, reinforcement class A400. Concrete of monolithic structures of the underground part of the building B25; W6; F50, class A400 fittings. External non-bearing walls - 250 mm thick, with floor-by-floor support on floor slabs, made of solid brick M100 on mortar M75, a layer of insulation and cladding with the "venfasad" system, partially with plastering. Structural solutions for fastening the ventilated facade system to the supporting structures of the building are developed in the working documentation in accordance with the current technical certificate of the Ministry of Regional Development of the Russian Federation. Pool structure - a monolithic reinforced concrete bowl (wall thickness 250 mm, bottoms - 250 mm), based on monolithic walls 250 mm thick, made of monolithic reinforced concrete B25; W8. Stairs - from monolithic reinforced concrete. The walls of the elevator shafts are monolithic reinforced concrete with a thickness of 150 mm. The load-bearing structures were calculated using the SCAD 11.3 software package, taking into account the joint work with the foundation. The relative mark of 0,000 corresponds to the absolute mark of 17.30. The foundation of the building was adopted on a natural basis in the form of a monolithic reinforced concrete slab 400 mm thick with a thickening (h=600 mm) in the areas of support of columns and walls, concrete B25; W6; F100. Under the foundation, concrete preparation is provided for a layer of crushed stone 150 mm thick. It is planned to replace the bulk soil under the foundation and install a sand cushion with layer-by-layer compaction with a thickness of 1,05 m. At the base of the sand cushion under the foundation slab there are plastic sandy loams EGE 2a (E = 16 MPa; e = 0,52; φ = 29 deg; c=18 kPa) and high-plastic loams IGE 2b (E=13 MPa; e=0,566; φ=25 deg; c=35 kPa). The project provides for pasting waterproofing of foundation surfaces in contact with the ground, and the installation of near-foundation drainage. The expected settlement of the building is 21 mm. A survey of the surrounding buildings was carried out. Buildings are assigned to the 3rd category of technical condition. The distance to the buildings of the surrounding buildings does not exceed the value of the depth of the compressible thickness.

 Technological solutions

 The set of polyclinic premises consists of the following main structural units located on floors 1-3 and the basement and made in their isolated structural zones: an adult polyclinic for 220 visits per shift with a day hospital for 14 beds; children's polyclinic for 120 visits per shift; department of geriatrics for 50 visits per shift; female consultation for 50 visits per shift; city ​​pharmacy; service and amenity premises, CSO premises, staff canteen with a hall for 20 seats, dressing rooms. The ground floor houses the premises of the CSO, staff wardrobes with separation of premises for children's and adult clinics. A pharmacy trading floor was designed with staff rooms, utility rooms and loading rooms. On the 1st floor there is a reception and vestibule group of premises for the structure of a children's and adult polyclinic in their isolated volumes, 2 separate blocks of premises for an infectious disease doctor with an isolated entrance (separately for children and adults), rooms and offices for women's consultations. The vestibule group of the children's department has separate entrances for receiving infants and 2 filter boxes for receiving other children. The geriatric department, located in a separate block, is represented by doctors' offices for the reception of elderly and senile people, an exercise therapy room, treatment rooms, rooms for social and medical workers. A swimming pool for children from 10 to 14 years old was designed for a group of children, consisting of 10 people, with all the necessary utility rooms. On the 1st floor there is a production area of ​​the pharmacy premises, consisting of the preparation of medicines in aseptic and under normal conditions. The pharmacy floors are connected by stairs and by means of a cargo lift. On the 2nd floor there are mainly offices of therapeutic departments, offices of doctors of narrow specializations, a dental office for adults and children, made in their isolated volumes separately for children's and adult clinics. A surgical department for a children's polyclinic was designed. On the 2nd floor, the premises of administrative and economic services are designed. On the third floor there is a day hospital for 14 beds for adults, a surgical department for adults, a department of rehabilitation treatment, rooms for functional diagnostics, and a radiology department. The premises of administrative and economic services and the CSO are common to all structural divisions. Rehabilitation treatment department, functional diagnostics rooms, radiodiagnostics department are common for all structural divisions of the facility and their visits are distributed over time.
